From the East Mountain High School Student Handbook:
The parent/guardian shall notify the school each day his or her child will be absent from any class and the reason for the absence; and shall provide a written and signed documentation showing that the absence is caused by illness, medical appointments, religious obligations, family situation or tragedy, or student safety, to be presented upon the first day a student returns from any absence. Failure to provide this verification results in unexcused absences. Please use the Attendance Verification Form for all absences. The form is available in the main office and on the EMHS website. The form may be faxed to 281-4173 Attn: Jennifer or emailed to firstname.lastname@example.org.
